Conditions: The agreement is for group travel only. A group is ten (10) or more passengers traveling together. In the event the group does not meet this minimum, or passengers do not travel with the group, they will be considered individual travelers and must be ticketed at the applicable fare (in which case all fare rules and blackout dates apply).

Deposit: To guarantee the fare and hold the number of seats, a deposit of $___ per seat must be received according to the applicable deposit deadline requirement determined by how far in advance you are booking your reservation. Payment must be made with one check or by credit card. Please note the Group ID number, record locator and departure date on your payment. Deposits will be refunded after tickets are issued or may be applied to the final ticket amount.

Deposit Utilization: __% utilization is required and will be determined by the number of seats reserved __ days prior to departure. For seats canceled in excess of __% at __ days prior or less, the $__ per seat deposit will be forfeited. If your group size falls below 10 passengers at __ days prior or less, the entire deposit will be forfeited.

Refunds: Full deposit refunds are available for cancellations __ or more days prior to departure. Refunds will be processed within __ weeks.

Final Payment/Ticketing: Passenger names and final payment must be received by the specified ticketing deadline. If final payment is not received or agency ticketing not completed as agreed, group space will auto-cancel. Once ticketed, applicable fees will apply for changes.

TICKETS ARE NON-REFUNDABLE.

Terms: Any changes to this agreement must be in writing and acknowledged by the parties. The airline reserves the right to change equipment or schedules. Should the airline cancel scheduled service to any city pair (origin and destination cities) in the group itinerary, the airline will return deposit amounts and paid fares without any further obligation or liability to you or the group. Airlines' only liability for delays or canceled flights caused by weather or other reasons beyond its control is to arrange for transportation on a later flight or, at the airlines' election, provide credit for other air transportation on the airline.

BY SENDING DEPOSIT AND/OR TICKETING, YOU AGREE TO THE TERMS AND CONDITIONS OF THIS AGREEMENT.
